Note the business and the part of the visit

Write down the business name, address or shopping center, and the exact area: an entrance, produce section, aisle, restroom, checkout lane, patio, food court, drink station, or pickup counter. If you have a receipt, order confirmation, or loyalty-app record, save it with your notes.

Think through the few minutes before the fall. Were you entering, waiting in line, carrying an item, leaving a restroom, or walking between tables? Those ordinary details help explain the setting without drawing conclusions about what caused the fall.

Describe what you observed in everyday language

You do not need special terminology. A plain description such as “my shoe slid on a clear liquid near the refrigerated cases” or “I caught my foot on a raised edge by the patio door” is easier to understand than a guess about policies or maintenance.

If employees were cleaning, stocking, serving, or working nearby, note what you personally observed. If you heard a statement, identify who said it and preserve the wording as accurately as you can.

  • Surface condition, including any liquid, debris, mat, or floor transition
  • Location of warning signs or whether you saw any
  • Names of staff, managers, or customers you spoke with
  • Photos of the area and any receipt or transaction record

Avoid filling gaps with assumptions

It is normal not to know every detail after a fall. Focus on what you remember and mark uncertainty honestly. A later conversation may identify additional information, but it is better not to state a conclusion you cannot support from your own experience.
